Announcing Primary Care at Partnership's Oshkosh Dental location: Open April 2, 2018

Hello Community Partners!

After a busy winter of renovations and planning with the community, we are ready to open our doors to provide primary care at our Oshkosh dental location. 

We are thrilled to be able to expand our services and link those in need to care.

We have lots of marketing materials to get out to you so please contact Julia.Garvey@thedacare.org or 920-882-6420 to get your supply!

Here are the details:

·        When will you open:                                 April 2, 2018. 

·        How many days a week?                         

    We will be open 2 days a week on Monday and Wednesday.

·        How do I make an appointment?         

   Call our main number at 920-731-7445 and ask to schedule at the Oshkosh location with Nurse Practioner. Jill Collier

·        Uninsured patients?                                  

    Partnership will assist with sliding fee scale and enrollment into BadgerCare and/or marketplace as eligible

·        All Patients Welcome!                               

   We do not have any county/city or specific designation for care.  Everyone is welcome!

·        Languages spoken at PCHC                    

   We speak English, Spanish and Hmong---Language line available for other world languages

Over the next few months our team will be doing some very exciting outreach, education and community work with many organizations in the greater Oshkosh area.
